Warning! Assumption of Risk. I am aware that spectators, observers, attendees, ice skating, ice hockey, camps, clinics, competitions, running/walking and general physical exercise collectively “the Activity” involves certain inherent risks, dangers and hazards, which can result in serious personal injury or death. I am aware that ice skating/hockey rinks, collectively the Pettit National Ice Center (“PNIC”) contain dangers that can cause serious injury or death. I hereby freely agree to assume and accept all known and unknown risks of injury or loss arising out of the Activity at the PNIC. I recognize and acknowledge that risks of the Activity can be greatly reduced by: taking lessons, abiding by the PNIC Rules, wearing protective equipment and using common sense. RELEASE AND WAIVER OF CLAIMS AGREEMENT: For allowing me to participate in the Activity at the PNIC, I agree, to the fullest extent permitted by law as follows: 1) TO WAIVE ALL CLAIMS that I have or may have against the PNIC arriving out of the Activity at the PNIC. 2) TO RELEASE the PNIC from the following, but not limited to: all liability for any loss, damage, injury or expense that I (or my next of kin) may suffer, arising out of the Activity, from any cause whatsoever, including negligence or breach of contract on the part of the PNIC in the operation, supervision, design or maintenance of the PNIC. By wearing this badge I give approval for my image and/or likeness to be photographed and/or taped to be used in promotional pieces by the PNIC.
